Influencing Factors on Discontinuation of LST and Use of Hospice: Focusing on the Patient¡¯s Autonomy

Abstract
This study validates the patient¡¯s autonomy in determining end-of-life care choices, specifically the life-sustaining treatment decision system and hospice¡¤palliative care. Analyzing data from 91,220 cancer patients, we found that older individuals without disabilities and higher income quintiles were more likely to opt for discontinuing life-sustaining treatment. Moreover, when patients made this decision by themselves, hospice utilization was 2.7 times higher than when the family decided. The probability of using hospice increased significantly with the patient autonomy, lower medical expenses, higher insurance premium quintiles, and residing in larger cities. These findings emphasize the importance of autonomy at the end of life care in preparing for a well-dying, highlighting a greater likelihood of opting for hospice and palliative care.
